Occupational Therapist (Bonus Eligible)

About the Role

Provide personalized in-home and community-based occupational therapy to older adults in various settings. Incorporate telehealth and virtual care to increase flexibility and patient access.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Deliver evidence-based therapy sessions to enhance mobility, independence, and overall well-being.
• Document patient progress accurately in electronic medical records, adhering to regulatory standards.
• Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.

Requirements

• Valid occupational therapy license or eligibility in practicing state(s).
• Degree from an accredited occupational therapy program.
• Proficiency with computers and EMR systems.
• Current CPR certification.
• Physical ability to perform patient transfers, repositioning, and extended walking.
• Ability to exert moderate physical effort and wear PPE as needed.
