About the Role
Provide personalized in-home and community-based occupational therapy to older adults in various settings. Incorporate telehealth and virtual care to increase flexibility and patient access.
Responsibilities
- Conduct evaluations and develop individualized treatment plans.
- Deliver evidence-based therapy sessions to enhance mobility, independence, and overall well-being.
- Document patient progress accurately in electronic medical records, adhering to regulatory standards.
- Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
Requirements
- Valid occupational therapy license or eligibility in practicing state(s).
- Degree from an accredited occupational therapy program.
- Proficiency with computers and EMR systems.
- Current CPR certification.
- Physical ability to perform patient transfers, repositioning, and extended walking.
- Ability to exert moderate physical effort and wear PPE as needed.
