Role Overview

As an Occupational Therapist in Home Health, you will assess and evaluate patients' functional status, activities of daily living, and occupational therapy needs based on medical conditions.

Key Responsibilities

• Provide services within state-defined scope of practice, following evidence-based techniques and collaborating with the healthcare team.
• Evaluate patients' functional abilities, document CMS requirements, and consult with physicians for care planning.
• Monitor and report patient responses to treatments and any condition changes to supervisors and physicians.
• Conduct comprehensive patient assessments using diagnostic and prognostic tests.
• Adapt to diverse home environments, regardless of age, race, creed, or disability.

Qualifications & Skills

• Current Occupational Therapy license in the practice state.
• Valid driver's license, vehicle insurance, or access to public transportation.
• Preferred: CPR certification, multitasking skills, independence, and strong communication and organizational abilities.
