Occupational Therapist Assistant - Home Health

About the role

We are seeking a Home Health Occupational Therapist Assistant to deliver personalized therapy services in patients' homes, under the supervision of a qualified Occupational Therapist. This role is focused on improving patients' strength, mobility, and quality of life through dedicated, community-based care.

Responsibilities

• Implement treatment plans developed by an Occupational Therapist
• Assist with rehabilitation activities and therapeutic exercises
• Design and adapt equipment to support patient independence
• Provide patient and family education on therapeutic techniques and safety
• Participate in care coordination and discharge planning
• Document patient progress and report outcomes to the supervising therapist
• Support community reintegration and promote functional independence

Requirements

• Current Occupational Therapist Assistant (OTA) licensure
• CPR certification
• Reliable transportation
• Adherence to TB screening and driver safety programs

Qualifications

Preferred experience includes at least six months as an occupational therapist assistant and prior home health experience.
