Nursing Assistant (NA) Care Transition Unit

Performs specific responsibilities under the direction and supervision of a licensed nurse, functioning within the framework of the NAI listing granted by the NC Department of Health and Human Services. Practices within the philosophy and goals of patient care services. May also serve in an Observer Role, monitoring assigned patients for a specified time period.

Responsibilities

• Participates in Quality Improvement (QI) Activities:
  • Engages in problem-solving and identifies opportunities for improvement.
  • Participates in data collection.
  • Follows safe practices, infection control, and fire/disaster regulations.
  • Contributes to cost savings activities.
• Participates in Education:
  • Attends and utilizes staff development programs and educational resources.
  • Fulfills annual mandatory educational requirements.
  • Assists in the orientation of new staff and students, as delegated by the supervising RN.
• Follows hospital and unit-based policies and procedures:
  • Provides quality patient care and maintains familiarity with job description and legal implications of role.
  • Maintains active NAI licensure/certification.
  • Builds interpersonal relations and communications by listening, giving/receiving feedback, and recognizing conflicts.
  • Works effectively in a team environment with respectful behaviors and attitudes.
  • Seeks help and utilizes chain of command appropriately.
• Makes unit, department, and hospital contributions:
  • Participates in unit goal setting and cost-effective use of equipment/supplies.
  • Assists in providing direct patient care and completes delegated tasks under licensed nurse supervision.
  • Monitors patients for escalating behaviors and alerts nurse if risk of harm is observed.
  • Maintains observation levels according to the patient’s plan of care.
  • Communicates effectively with the care team, including receiving report and providing handoff.
  • Documents accurately in the medical record and provides hazard surveillance.
  • Engages patients in distraction activities as needed.

Requirements

• High School diploma or GED (preferred).
• Active NAI listing with NC Department of Health and Human Services (required).
• Current American Heart Association HeartSaver or higher certification (required).
• De-escalation training (e.g., Non-violent Crisis Intervention) required within 90 days of hire (not required for Ambulatory clinical positions).
