About the Role

The Transitional Care Unit at M Health Fairview University of Minnesota Medical Center - West Bank Campus is dedicated to providing exceptional care to patients recovering from acute illnesses or surgeries. The unit serves a diverse range of diagnoses including neurology, orthopedics, oncology, amputee care, spinal cord injury, transplant patients, and others. The unit is highly collaborative, with interdisciplinary teams working closely with medical doctors, physiatrists, rehabilitation nurses, social workers, dietitians, and others to facilitate patient recovery and return home.

Responsibilities

• Partners with patient care givers in care/decision making.
• Performs personal grooming, bathing, toileting.
• Feeds patients and assists with oral hygiene.
• Collects and tests specimens.
• Assists with patient mobility and changing linens.
• Answers call lights promptly and reports unusual findings to the RN.
• Aids in admissions, transfers, and discharges of patients.
• Orients patients and families to the environment and ensures belongings are accounted for.
• Transports patients, equipment, specimens, and supplies.
• Ensures patients arrive at appointments on time.
• Uses transfer belts for patient transfers and ambulation.
• Cleans and sets up rooms and equipment.
• Orders and stocks supplies.
• Documents tasks performed accurately and on appropriate forms.

Requirements

• Current on the MN CNA Registry.
• BLS (Basic Life Support) for Healthcare Providers.

Qualifications

• High school diploma or equivalent.
• Current Minnesota Certified Nursing Assistant (CNA) license.
• Experience in a hospital setting preferred.

Skills

• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of patient care procedures and safety protocols.

Pay

The posted compensation range corresponds to the minimum and maximum pay rates outlined in your union agreement for the respective job classifications at the time of job posting. An individual's pay rate within this range may be influenced by various factors including FTE, skills, knowledge, educational background, and qualifying experience as specified in the contract. If your role is eligible for a sign-on bonus, the bonus program that is approved and in place at the time of offer, is what will be honored.
