Nurse Practitioner/Physician Assistant

About the Role

Join our team of experts in rehabilitation at Total Extended Care Services, a growing Physical Medicine and Rehab group. In this role, you will partner with our PM&R physician in the sub-acute rehab setting, working in-person at skilled nursing facilities. This position is ideal for those passionate about improving patient outcomes and collaborating within an interdisciplinary, specialized team.

Location: In-person at skilled nursing facilities
Schedule: Flexible options, part-time positions available

Responsibilities

• Partner with the facility's therapy department to provide the best possible rehab care.
• Communicate and collaborate with the patient's primary care team to ensure the best outcomes.
• Complete timely charting and documentation (can be done from home).

Requirements

• Licensed Nurse Practitioner (Required)
• Certified Nurse Practitioner (Required)
• Licensed Physician Assistant (Required)
• Certified Physician Assistant (Required)
• Must be licensed in Michigan
• Reliable transportation
