About The Role
Location: In-person at skilled nursing facilities. Schedule: Flexible options, Part-time positions available. Culture: Detail-oriented, outcome-focused, stable, people-oriented, and team-oriented.
Responsibilities
- Partner with the facility's therapy department to provide the best possible rehab care.
- Communication and collaboration with the patient's primary care team to ensure the best outcomes.
- Timely charting/documentation is required but can be done from home.
