About the Role
Join our team of experts in rehabilitation at skilled nursing facilities in Michigan. This in-person, part-time position offers flexible scheduling and a detail-oriented, outcome-focused, stable, and team-oriented culture.
Responsibilities
- Partner with the facility's therapy department to provide the best possible rehab care.
- Communicate and collaborate with the patient's primary care team to ensure optimal outcomes.
- Complete timely charting and documentation (can be done from home).
Requirements
- Licensed Nurse Practitioner (Required)
- Certified Nurse Practitioner (Required)
- Licensed Physician Assistant (Required)
- Certified Physician Assistant (Required)
- Must be licensed in Michigan
- Reliable transportation
