About the role
Full-time, 100 % outpatient position with office hours from 8 am to 5 pm, Monday through Friday. The role focuses on a geriatric patient population.
Responsibilities
- Deliver patient care through assessment, evaluation, diagnosis, recognition and prevention of adverse events, planning, and implementation.
- Perform duties in four practice areas: clinician, teacher, leadership, and professional development.
- Collaborate with other disciplines to ensure safe, effective, and efficient patient care and desired outcomes.
- Continuously monitor and improve quality of care through customer service and performance improvement initiatives.
- Provide leadership within the practice location.
- Function in accordance with Florida law.
- Treat less urgent acute conditions, manage chronic conditions, and coordinate care.
Requirements
- Graduate from an accredited Nurse Practitioner or Physician Assistant program.
- State of Florida license.
- Preferably 3 years of experience in a primary care setting.
- Managed Care, Medicare Advantage, and HEDIS coding experience.
- Bilingual (English/Spanish) strongly preferred.
