Nurse Practitioner / Physician Assistant

We are seeking a full-time primary care Nurse Practitioner or Physician Assistant for an 100% outpatient position with office hours from 8 am to 5 pm, Monday through Friday. The ideal candidate will be comfortable caring for a geriatric patient population.

Responsibilities

• Deliver patient care through assessment, evaluation, diagnosis, recognition and prevention of adverse events, planning, and implementation.
• Perform responsibilities in four areas of practice: clinician, teacher, leadership, and professional development.
• Collaborate with other professional disciplines to ensure safe, effective, and efficient patient care delivery and achievement of desired patient outcomes.
• Continuously monitor quality patient care through excellence in customer service and performance improvement that demonstrates positive outcomes in patient care.
• Provide leadership in the practice location, functioning consistent with Florida law.
• Treat less urgent acute conditions, manage chronic conditions, and provide care coordination.

Requirements

• Graduate from a Nurse Practitioner or Physician Assistant program.
• Preferably 3 years of experience working in a primary care setting.
• State of Florida license required.
• Bilingual (English/Spanish) strongly preferred.
• Managed Care, Medicare Advantage, and HEDIS coding experience.
