About the role

As a Nurse Practitioner (NP) or Physician Assistant (PA) at CenterWell Senior Primary Care, you'll be part of a collaborative, versatile team including physicians, pharmacists, care coaches, and behavioral health specialists. Together, you'll deliver personalized, high-quality care to seniors.

Responsibilities

  • Provide comprehensive care to seniors following clinical standards.
  • Collaborate daily with your care team to ensure coordinated, high-quality care.
  • Manage referrals, hospital/SNF coordination, DME, and home health services.
  • Participate in daily huddles (CAs tend to lead the huddles).
  • Document care with support from quality-based coders.
  • Participate in on-call rotation and support growth projects at your center.
  • Ensure compliance with all licensing and accreditation requirements.

Requirements

  • Active, unrestricted Nurse Practitioner or Physician Assistant license in the appropriate state.
  • Bachelor’s degree.
  • 8 or more years of technical experience.
  • 2 or more years of project leadership experience.
  • Must participate in Humana/Senior Bridge’s Tuberculosis (TB) screening program if selected.
  • Passionate about contributing to an organization focused on improving consumer experiences.
  • Willing to participate in limited on-call coverage by phone.

Preferred Qualifications

  • Master’s in Science of Nursing or Physician Assistant studies.

Pay

The starting base pay range for this full-time (40 hours per week) role is $123,800 - $170,300 per year. The pay range may vary based on geographic location and individual qualifications such as skills, knowledge, experience, education, and certifications. This role is also eligible for a bonus incentive plan based on company and/or individual performance.

Schedule

Scheduled weekly hours: 40.
