Nurse Practitioner/Physician Assistant

About the role
As a Nurse Practitioner (NP) or Physician Assistant (PA) at CenterWell Senior Primary Care, you'll be part of a collaborative, versatile team including physicians, pharmacists, care coaches, and behavioral health specialists. Together, you'll deliver personalized, high-quality care to seniors.
Responsibilities
- Provide comprehensive care to seniors following clinical standards.
- Collaborate daily with your care team to ensure coordinated, high-quality care.
- Manage referrals, hospital/SNF coordination, DME, and home health services.
- Participate in daily huddles (CAs tend to lead the huddles).
- Document care with support from quality-based coders.
- Participate in on-call rotation and support growth projects at your center.
- Ensure compliance with all licensing and accreditation requirements.
Requirements
- Active, unrestricted Nurse Practitioner or Physician Assistant license in the appropriate state.
- Bachelor’s degree.
- 8 or more years of technical experience.
- 2 or more years of project leadership experience.
- Must participate in Humana/Senior Bridge’s Tuberculosis (TB) screening program if selected.
- Passionate about contributing to an organization focused on improving consumer experiences.
- Willing to participate in limited on-call coverage by phone.
Preferred Qualifications
- Master’s in Science of Nursing or Physician Assistant studies.

Pay
The starting base pay range for this full-time (40 hours per week) role is $123,800 - $170,300 per year. The pay range may vary based on geographic location and individual qualifications such as skills, knowledge, experience, education, and certifications. This role is also eligible for a bonus incentive plan based on company and/or individual performance.
