About the Role
A Nurse Practitioner or Physician Assistant will provide evidence-based, personalized care for adult patients with obesity, Type 2 diabetes, and related metabolic conditions within a multidisciplinary team.
Responsibilities
- Conduct comprehensive evaluations and develop individualized treatment plans, including lifestyle modifications and medication management.
- Monitor patient progress through routine follow-ups, adjusting care plans to ensure long-term success.
- Provide education on nutrition, physical activity, and behavioral strategies.
- Collaborate with dietitians, behavioral health specialists, pharmacists, and physicians for coordinated care.
- Order, interpret diagnostic studies, and manage comorbid conditions such as diabetes and hypertension.
- Maintain accurate clinical documentation following organizational standards.
Practice Highlights
- Participate in a multidisciplinary, evidence-based, structured, longitudinal care model.
- Support metabolic health through comprehensive services, including nutrition therapy and behavioral health.

Requirements
- Must hold an active license and registration as a Physician Assistant or Nurse Practitioner in New York State.
- Physician Assistants: relevant degree, certification, and ambulatory care experience.
- Nurse Practitioners: qualifying education, certification, pharmacology coursework, and preferred ambulatory care experience.
- CPR certification required for both roles.
