Nurse Practitioner or Physician Assistant - St. Catherine of Siena Hospital - – Hospital Medicine Specialty Team

About the role

GlyCare’s providers play a critical role in improving patient safety, reducing complications, and supporting hospitals through specialized inpatient diabetes management. Our work intersects with nearly every major hospital specialty, including: Cardiology, Nephrology, Infectious Disease, Surgery, Nutrition, Wound Care, Rehabilitation.

Responsibilities

• Provide inpatient diabetes and glycemic management consultations
• Manage adult hospitalized patients on Med-Surg and Step-Down units
• Adjust insulin and diabetes therapies using evidence-based protocols
• Collaborate with hospitalists, nursing, pharmacy, and acute care teams
• Document directly within the hospital EMR
• Practice within a highly collaborative inpatient consultative model

Requirements

• Active, unrestricted New York NP or PA license
• NP certification as FNP, AGNP, or ACNP, or PA-C certification
• EMR proficiency required
• Recent Inpatient experience required
• DEA not required
• Minimum one-year commitment required

• Strong new graduates with inpatient clinical training are also encouraged to apply

Skills

Must have strong communication skills, problem-solving abilities, and the ability to work effectively in a multidisciplinary team setting.

$105,000 - $116,000 annually
