Responsibilities

• Work in a care team setting in collaboration with other team members including physicians and other allied health staff.
• Be involved in population management strategies to improve chronic illness outcomes.
• Provide care including but not limited to comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments, non-pharmacologic therapies, and counseling patients.

Qualifications

• Nurse Practitioner: Masters prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current Minnesota RN and APRN license and certification as an FNP. DNP is preferred.
• Physician Assistant: Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the Minnesota State Board of Medical Practice. DMSc is preferred.

Additional Qualifications

• 3-5 years of NP or PA experience in an outpatient primary care setting preferred.
• Previous experience working with pediatric as well as adult patients preferred.
• Mayo Clinic Health System experience preferred.

• $57.63 - $80.42 / hour
