About the role
Join a team providing in-home comprehensive health assessments to improve patient outcomes and close care gaps. The role involves conducting up to one-hour assessments in members' residences, including homes and Skilled Nursing Facilities. You will review medical history, medication adherence, social factors, and perform point-of-care testing while collaborating with physicians, case managers, and caregivers to deliver personalized care.
Responsibilities
- Perform in-home comprehensive health assessments and document findings
- Provide health education to patients
- Order tests and collaborate with healthcare team members
- Support patient follow-up and referrals to optimize care
Requirements
- Master’s Degree or equivalent with NP licensure and prescriptive authority
- Board certification in Adult, Gerontology, Family, Emergency, or Acute Care
- Current BLS, ACLS, or CPR certification
- At least 1 year of NP experience
- Ability to travel extensively, including overnight stays
- Valid driver’s license and insurance
- Strong computer skills and excellent communication abilities
