About the role
Join a healthcare provider delivering comprehensive in-home health assessments to improve patient outcomes and reduce costs. As a Nurse Practitioner (NP), you will conduct up to hour-long visits, assessing medical history, medication adherence, social factors, and risks, often in members' homes or facilities.
Responsibilities
- Perform comprehensive health assessments
- Order diagnostic tests
- Collaborate with physicians, case managers, and caregivers
- Provide patient education and follow-up
Qualifications
- Master’s Degree or equivalent experience
- Current NP licensure with prescriptive authority
- Board certification in relevant specialty
- BLS/ACLS/CPR certification
- At least 1 year of NP experience
- Valid driver’s license and ability to travel extensively
- Strong communication and computer skills
- Familiarity with medical records systems
