Nurse Practitioner - Full Time Regional Traveler

About the Role

We are seeking a Nurse Practitioner (NP) to perform comprehensive in-home health assessments for adult and geriatric patients. The role involves evaluating a member’s medical history, medication adherence, social environment, and health risks during visits that may last up to one hour.

Responsibilities

• Conduct in-home health assessments for adult and geriatric patients.
• Evaluate medical history, medication adherence, social environment, and health risks.
• Order diagnostic tests as needed.
• Collaborate with healthcare teams to coordinate patient care.
• Provide patient education on health management and preventive care.
• Document findings and assessments per established protocols.

Requirements

• Master’s Degree in Nursing or equivalent.
• Current Nurse Practitioner licensure with prescriptive authority.
• Board certification in a relevant specialty (e.g., Family, Adult, Gerontology).
• BLS, ACLS, and/or CPR certification.
• Minimum of one year of Nurse Practitioner experience.
• Ability to travel extensively for in-home visits.
• Strong communication and computer skills.
