About the role

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner) to provide In-Home Health Evaluations. In this role, you’ll engage with people in the comfort of their own homes, helping bridge gaps in care. You will primarily travel—up to 100% of the time—across a defined coverage area, which may require overnight hotel stays, car rentals, and/or flights. Multiple state licenses will be required as dictated by business needs.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams, spirometry, and more. You’ll use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter treatment regimens.

As an in-home clinician, your primary workplace is the private residence of health plan members. These environments may present diverse conditions, including household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be comfortable and safe performing duties in such settings.

Responsibilities

  • Conduct in-home health evaluations, building trusted relationships to improve health outcomes.
  • Travel locally and within licensed states, with flexibility for overnight stays and car rentals/flights as needed.
  • Perform 45-minute visits, including medication reviews, physical evaluations, and diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screenings).
  • Use an iPad and clinical workflow to document evaluations without altering treatment regimens.
  • Conduct virtual visits as required.
  • Provide all Signify Health services, including Diagnostic & Preventive Services products.

Requirements

  • Active, unrestricted Nurse Practitioner license(s) in coverage area(s).
  • Board certification (required).
  • Able to obtain and maintain multi-state licenses as dictated by business needs.
  • Ability to cover all counties within a state and flex to other areas as needed.

Pay

The typical pay range for this role is $78,331.00 - $168,714.00 annually, depending on experience, education, geography, and other relevant factors.

Schedule

  • Full-time position with anticipated weekly hours of 40.
  • Flexibility to travel locally and within licensed states, including overnight stays and virtual visits as needed.
