Nurse Practitioner - Full Time Regional Travel

Role Overview

The provider conducts comprehensive in-home health assessments, lasting up to one hour, evaluating medical history, current health, medication adherence, social factors, and risks to improve care quality and address care gaps. They collaborate with physicians, social workers, family, and other healthcare professionals, administer tests, and document findings following protocols. Services are delivered in members' homes or facilities.

Responsibilities

• Performing adult/geriatric assessments including diagnosis, treatment, and health education
• Administering and ordering point-of-care and lab tests
• Collaborating with healthcare team and primary care physicians for follow-up and education
• Providing services at members' residences

Qualifications & Skills

• Master's Degree or equivalent with NP licensure and prescriptive authority
• Board certification in relevant adult, family, or acute care specialties
• Current BLS, ACLS, or CPR certification
• At least 1 year of NP experience
• Able to travel extensively, including overnight stays
• Valid driver's license and appropriate automobile insurance
• Strong computer skills and excellent communication abilities
