Nurse Practitioner - Full Time Regional Traveler

About the role

The role involves conducting comprehensive in-home health assessments for diverse patients, including older adults and at-risk individuals. This includes reviewing medical history, medication adherence, social factors, and performing point-of-care testing during visits that can last up to an hour. Providers collaborate with physicians, case managers, and caregivers to improve health outcomes and close care gaps.

Responsibilities

• Conduct comprehensive in-home health assessments for diverse patients, including older adults and at-risk individuals.
• Review medical history, medication adherence, social factors, and perform point-of-care testing during visits lasting up to an hour.
• Collaborate with physicians, case managers, and caregivers to improve health outcomes and close care gaps.

Requirements

• At least 1 year of experience as a Nurse Practitioner.
• Current licensure as a Nurse Practitioner.
• Strong communication and computer skills.
• A valid driver's license.

Qualifications

• Bachelor's degree in Nursing or related field.
• Master's degree in Nursing or related field preferred.
• Current certification as a Nurse Practitioner.
