About the role
We are seeking a Nurse Practitioner to deliver high-quality, equitable primary care to adult Medicare patients in an outpatient setting.
Responsibilities
- Independent patient assessments
- Care coordination
- Chronic disease management
- Health education
- Participation in population health initiatives
- Conducting office visits
- Wellness exams
- Health risk assessments
- Home visits when needed
Requirements
- Hold an active NP license
- National certification in relevant specialties
- DEA authorization
Qualifications
- Primary care experience preferred
- Familiarity with HCC and ICD-10 coding preferred
- Bilingual skills preferred
