Nurse Practitioner, Bilingual (Spanish)

Role: Nurse Practitioner, Advanced Practice Provider Responsibilities Provide effective, equitable, value-based primary care to adult Medicare patients, fostering long-term relationships and trust.Conduct office visits for routine, acute, and wellness assessments, ensuring comprehensive documentation.Coordinate care with specialists, testing facilities, and other providers to optimize patient outcomes.Lead population health initiatives, ensuring adherence to preventive screenings and health protocols.Assist with phone triage, outreach, patient education, and conduct home visits as needed.Participate in promotional activities and other assigned duties. Qualifications & Skills Master of Science in Nursing, Doctor of Nursing Practice, or NP certification.Active, non-probationary state Nurse Practitioner license and DEA license.Certification in Family, Adult-Gerontology, or Gerontological Nurse Practice.Preferred experience in primary care, understanding of HCC, ICD-10 coding, and health assessments.Strong teamwork, communication, and patient education skills. Bilingual skills are a plus.
