About the role
We are seeking a Nurse Practitioner, Advanced Practice Provider to deliver effective, equitable, and value-based primary care focused on adult Medicare patients. The role reports to the Center Medical Director and involves close collaboration with operational leadership, focusing solely on primary care, care coordination, and population health within an outpatient setting.
Responsibilities
- Conduct routine and acute office visits
- Administer wellness exams
- Coordinate care with specialists
- Lead population health initiatives
- Educate patients
- Participate in outreach
- Conduct home visits as needed
Requirements
- Master of Science in Nursing or Doctor of Nursing Practice (DNP)
- National certification in Family, Adult-Gerontology, or Gerontological Nurse Practitioner
- Active NP license
- DEA license
- US work authorization
Skills
- Primary care experience
- Familiarity with HCC documentation
- ICD-10 coding proficiency
- Bilingual skills

Salary range: $87,035 – $206,206.
