What You'll Do

  • Deliver comprehensive primary care services across the lifespan.
  • Perform preventive care, wellness visits, and chronic disease management.
  • Diagnose and treat acute and chronic medical conditions.
  • Independently manage an established patient panel.
  • Collaborate closely with behavioral health, care management, and support teams.
  • Participate in quality improvement and population health initiatives.
  • Document patient care accurately within the electronic health record.
  • Help foster a culture of teamwork, compassion, and continuous improvement.

Required Experience & Qualifications

  • Nurse Practitioner (APRN): Current New Hampshire APRN and RN licensure, Board Certification through ANCC and/or AANP, Current DEA registration, Current BLS certification.
  • Physician Associate (PA): Current New Hampshire Physician Associate license, NCCPA Certification, Current DEA registration, Current BLS certification.

Experience Candidates must have a minimum of one (1) year of independent outpatient clinical practice experience. Providers with three to five (3–5) years of independent primary care experience are strongly encouraged to apply. Demonstrated experience independently managing an established primary care patient panel. Experience providing comprehensive primary care services across the lifespan. Experience diagnosing and managing acute and chronic medical conditions across the lifespan. Primary Care, Family Medicine, Internal Medicine, Community Health, Urgent Care, or FQHC experience is strongly preferred. Strong commitment to team-based care, quality outcomes, and population health management.
