Responsibilities
- Perform PM&R consults and follow-up care at skilled nursing facilities independently and in collaboration with supervising physician
- Complete accurate and timely clinical documentation
- Review patient history, medications, and treatment plans
- Care coordination with facility staff, therapists, and supervising physician
- Ensure compliance with clinical and documentation standards
Qualifications
- Active Florida APRN or PA license
- Current DEA license (or eligibility)
- Experience in post-acute care, PM&R, orthopedics, neurology, pain management, or geriatrics preferred
- Strong clinical reasoning and documentation skills
- Able to work effectively within a collaborative physician-led practice
- Reliable transportation for facility rounding
