About the role

The Nurse Navigator acts as a liaison between patients, physicians, and the respective care team. They coordinate care across the continuum, provide education and support, facilitate decision-making, advocate for patients, and improve throughput and patient satisfaction.

Responsibilities

• Coordinate care across the continuum, including hospital, managed care, and medical office settings.
• Provide education and support to patients and their significant others throughout the continuum of care.
• Facilitate decision-making processes and advocate for patients.
• Monitor patient outcomes and participate in quality improvement activities.
• Work in a multidisciplinary setting, collaborating with healthcare team members to positively impact patient outcomes and experiences.

Qualifications

• Associate’s Degree in Nursing (ASN).
• Registered Nurse (RN) license.
• Must earn a Bachelor of Science in Nursing (BSN) within three years of hire.
• Three years of clinical experience working in a hospital, managed care environment, or medical office.
• Experience in Electronic Medical Record systems preferred.
• Bilingual preferred.
• Preferred certification in area of expertise.
• BLS required for Boca Regional only.

Pay

The estimated salary range for this position is $80,516.80 - $104,671.84 per year, depending on experience.
