NEUROSCIENCE NURSE NAVIGATOR

About the role

The Neuroscience Nurse Navigator is a registered nurse (RN) who partners with and advocates for patients/families/significant others to provide physical, emotional and spiritual support which impacts patient outcomes.

Responsibilities

• See that patients receive treatment services that are timely, appropriate and comprehensive.
• Identify patients’ needs and work collaboratively with a multi-disciplinary team across the in-patient and out-patient continuum of care.
• Support patient/family education, monitor the recommended treatment plan, ensure appropriate follow-up in the outpatient setting, and partner with community resources as guided.
• Monitor identified quality measures and work with the multi-disciplinary team to reach expected outcomes.
• Accountable for compliance with the Ohio Nurse Practice Act, Nursing Code of Ethics and applicable regulatory standards.

Requirements

• Minimum Level of Education Required: Bachelor's degree
• Licensure/Certification/Registration: Valid Ohio RN license, current CPR certification required at the time of hire. Certification in area of specialty preferred
