Position Overview
As the Nurse Navigator, you will play a vital role in coordinating the care for our patients, developing personalized care plans in collaboration with patients and their primary care providers. Your responsibilities will include:
- Collaborating with physicians, nurses, and other healthcare professionals to ensure timely delivery of tests and treatments
- Advocating for patients and balancing their care needs with financial considerations
- Educating patients about the pain program; providing scheduled education to patients on a multitude of factors that affect their chronic pain
- Triaging referred patients
- Managing care delivery across the patient care continuum for new patients
- Working with DME processing and managing of orders
- Collaborating with and facilitating services to meet the needs of the Limb Loss Clinic
Qualifications
- Requires an Associates of Nursing (ASN)
- Bachelor of Nursing (BSN) preferred
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license
- Requires that the RN has graduated from a nationally accredited nursing program
- 3-5 years of experience preferred
- Requires Basic Life Support certification through the AHA
- Other advanced life support certifications may be required per unit/department specialty according to patient care policies
- Specialty certifications may be required per unit/department specialty in accordance with the department provision of care
- Requires knowledge of personal computers using various types of word processing and database software
- Requires clinical work experience, medical terminology
- Requires understanding of medical record requirements, regulations and policies
- Experience with education and navigating complex conditions preferred
