About the role

The Nurse Navigator acts as a liaison between patients, physicians, and the respective care team. They are responsible for coordinating care across the continuum, providing education and support to patients, facilitating decision-making, advocating for patients, and ultimately improving throughput and patient satisfaction.

Responsibilities

  • Coordinate care across the continuum, ensuring seamless transitions between settings.
  • Provide education and support to patients and their significant others throughout the continuum of care.
  • Facilitate decision-making processes and advocate for patients to ensure optimal care.
  • Monitor patient outcomes and participate in quality improvement activities.
  • Collaborate with healthcare team members to positively impact patient outcomes and experiences.

Requirements

  • Bachelor Science Nursing (BSN).
  • Basic Life Support (BLS) certification.
  • Registered Nurse (RN) license.
  • Advanced Cardiovascular Life Support (ACLS) certification.
  • Three years of clinical experience in a hospital, managed care environment, or medical office.
  • Proficient in computer applications and typing skills.
  • Experience in Electronic Medical Record (EMR) systems preferred.
  • Bilingual proficiency preferred.
  • PREFERRED: Certification in area of expertise.
  • For South Entities, RNs hired after 2/2012 with an Associates degree must complete the BSN degree within three years.

Pay

The estimated salary range for this position is $80,516.80 - $107,087.34 per year, depending on experience.
