About the role
The Nurse Navigator acts as a liaison between patients, physicians, and the respective care team. They are responsible for coordinating care across the continuum, providing education and support to patients, facilitating decision-making, advocating for patients, and ultimately improving throughput and patient satisfaction.
Responsibilities
- Coordinate care across the continuum, ensuring seamless transitions between settings.
- Provide education and support to patients and their significant others throughout the continuum of care.
- Facilitate decision-making processes and advocate for patients to ensure optimal care.
- Monitor patient outcomes and participate in quality improvement activities.
- Collaborate with healthcare team members to positively impact patient outcomes and experiences.
Requirements
- Bachelor Science Nursing (BSN).
- Basic Life Support (BLS) certification.
- Registered Nurse (RN) license.
- Advanced Cardiovascular Life Support (ACLS) certification.
- Three years of clinical experience in a hospital, managed care environment, or medical office.
- Proficient in computer applications and typing skills.
- Experience in Electronic Medical Record (EMR) systems preferred.
- Bilingual proficiency preferred.
- PREFERRED: Certification in area of expertise.
- For South Entities, RNs hired after 2/2012 with an Associates degree must complete the BSN degree within three years.

Pay
The estimated salary range for this position is $80,516.80 - $107,087.34 per year, depending on experience.
