Nurse Navigator 2, MNI NeuroPsych - Office, FT, 8:00am - 4:30pm

About the role

The Nurse Navigator (NN) serves as a multidisciplinary team leader and liaison between patients, family, physicians, clinical care teams, community organizations, and BHSF leadership. They coordinate care across the continuum, provide education, support, and resources to patients and families, facilitate shared decision-making, advocate for patients, ensure timely access to quality multidisciplinary care, and enhance patient satisfaction.

Requirements

• Bachelor’s degree in Nursing
• Advanced Cardiac Life Support (ACLS)
• Basic Life Support (BLS)
• Neonatal Resuscitation Program (NRP)
• Pediatric Advanced Life Support (PALS)
• Registered Nurse License
• Certification in area of expertise required
• Five years nursing experience in key relevant areas: MedSurg, Oncology Care, Patient Navigation, or specific Disease Sites (e.g., Breast, GI, GU, Neuro)
• Skilled in Microsoft Office, EMR applications (Word, Excel, Sharepoint, TEAMS, HEDIS, STAR, etc.) and typing
• Bilingual English/Spanish preferred

Qualifications

• Bachelor of Science in Nursing (BSN)
• Experience working in a hospital, managed care, or medical office preferred

Skills

• Strong communication and interpersonal skills
• Ability to manage multiple tasks and prioritize effectively
• Knowledge of healthcare policies and procedures
• Proficiency in Microsoft Office and EMR applications
