Nurse Navigator 2, MNI Office, FT, 8:00am - 4:30pm

About the role

The Nurse Navigator acts as a liaison between patients, physicians, and the respective care team. They are responsible for coordinating care across the continuum, providing education and support to patients, facilitating decision-making, advocating for patients, and ultimately improving throughput and patient satisfaction.

Responsibilities

• Coordinate care across the continuum, ensuring seamless transitions for patients
• Provide education and support to patients and their significant others
• Facilitate decision-making processes and advocate for patient needs
• Monitor patient outcomes and participate in quality improvement activities
• Collaborate with healthcare team members to positively impact patient outcomes and experiences

Requirements

• Bachelor’s degree in Nursing (BSN)
• Active Registered Nurse (RN) license
• Three years of clinical experience in a hospital, managed care environment, or medical office
• Knowledge of HEDIS and Star ratings, risk adjustment, and HCC coding (preferred)
• Proficient in computer applications and typing skills
• Experience in Electronic Medical Record systems (preferred)
• Bilingual (preferred)
• Preferred certification in area of expertise

Qualifications

• Neonatal Resuscitation Program (NRP)
• Pediatric Advanced Life Support (PALS)
• Basic Life Support (BLS)
• Advanced Cardiovascular Life Support (ACLS)

Pay

The estimated salary range for this position is $80,516.80 - $107,087.34 per year, depending on experience.
