About the role
The Diabetes Nurse Educator is a vital member of a multidisciplinary care team, responsible for supporting patients through education, intervention, and clinical coordination in diabetes management.
Responsibilities
- Assess patients for diabetes-related complications
- Educate on self-management and lifestyle changes
- Develop group and individual education programs aligned with ADA guidelines
- Monitor intervention effectiveness
- Instruct on medication and device usage including insulin, CGMs, and pumps
- Interpret blood glucose logs
- Assist with device troubleshooting
- Perform CGM sensor downloads for provider review
- Collaborate with families and interdisciplinary teams across care settings
- Ensure compliance with health standards
- Promote diabetes awareness through community outreach
- Stay current with clinical best practices via ongoing education
- Support quality initiatives in diabetes care
Qualifications
- Minimum 2 years healthcare experience with proficiency in RN curriculum areas
- Bachelor's Degree in Nursing
- NY State RN license
- Current CPR certification
- Certified Diabetes Educator within 2 years of hire
Preferred Skills & Environment
- Experience in primary or specialty care
- Valid driver’s license
- Additional certifications are advantageous
Work is performed in an office setting, requiring mobility, handling equipment, and adherence to infection control protocols, serving patients of all ages.
