Nurse Coordinator

About the role

This position involves managing clinic patients' care, integrating cultural practices and beliefs, and coordinating with multidisciplinary teams.

Responsibilities

  • Guide and assist patients through the healthcare system, including assistance with appointments and treatment decisions.
  • Follow patients throughout the care continuum, collaborating with inpatient, outpatient, and community care management resources.
  • Act as an essential link between the primary care provider and the entire care team for the patient.
  • Establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, and radiation oncology staff).
  • Ensure patients' needs and preferences are met throughout the treatment process, serving as a patient advocate.
  • Maintain patient progress and provide ongoing support.
  • Work on the multidisciplinary team as an advocate, educator, and counselor for clinic patients.
  • Participate in screening initiatives and community events, and coordinate with other tribal departments.
  • Update knowledge and skills through in-service programs, continuing education, and self-study.
  • Adhere to applicable regulations, policies, and procedures.

Requirements

  • Graduation from an accredited Registered Nurse (RN) program and active Washington State RN license.
  • Five (5) years of progressively responsible nursing experience.
  • Two (2) years in care coordination, case management, or population health.
  • Two (2) years in a primary care, ambulatory, or community health setting.
  • Current Basic Life Support (BLS) certification at time of hire.
  • Valid Washington State driver’s license.

Knowledge

  • Knowledge of acute illness and chronic disease processes and management.
  • RN scope of practice techniques and methods.
  • Medications and treatments used in patient care.
  • Infection control procedures.
  • Mental or behavioral characteristics of physically or mentally disabled people.
