About the role
Citizen Care Inc., a subsidiary of Partners For Quality Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit organization, accredited by the Council on Quality & Leadership. Citizen Care has locations throughout the West Hills and South Hills of Allegheny County.
Responsibilities
Responsible to Medical Coordinator for providing and overseeing medical care to people with intellectual and other developmental disabilities through the following:
- Supervision of the implementation of medical supports and services
- Communicating health care concerns/needs to the appropriate people
- Focus on Performance Based Contracting standards and The Council on Quality Leadership (CQL) standards
- Compliance with the applicable Regulations & State initiatives such as the Fatal 5
- Oversight of Health Risk Screening Tool (HRST) process
- Serve as the agency's Clinical HRST rater and reviewer
- Oversight of development / implementation of Electronic Health Records (EHR)
- Input into use and implementation of outside agency's health records, i.e. Epic
- Exploration of additional collaboration/ communication with other medical facilities
- Input into training development related to medical needs of individuals supported
- Lead investigator/consultant for deaths or issues involving medical diagnosis
Qualifications
- Must be a Registered Nurse (RN) or LPN with a current Pennsylvania license
- Four (4) years experience in nursing required
- Experience in Intellectual Disabilities and mental health services desired
- Experience in dealing with emergencies and/or acute medical problems
- Valid driver's license
- Use of personal vehicle required
- Act 34 and (if applicable) FBI Clearances required
- Excellent references, both personal and work
- Current PA RN or LPN license
- Complete and maintain ODP's Medication Administration Supervisor certification
- Valid PA driver's license with acceptable driving record and use of personal vehicle
Pay
- Minimum Compensation USD $30.00/Hr.
- Maximum Compensation USD $34.00/Hr.
Schedule
- Full-Time; 40 hours / week
