About the Role
Provides in-home clinical care to high-risk patients as part of an interdisciplinary team, focusing on preventive, acute, and chronic condition management. Conducts comprehensive assessments, including medical history, vital signs, and screenings. Manages patient care during acute episodes, sub-acute rehabilitation, and advanced care planning. Ensures accurate documentation and collaborates with team members. Provides patient and caregiver education and counseling.
Responsibilities
- Travel statewide up to 85%, with potential additional incentives.
- Perform physical exams, screenings, and diagnostics.
- Manage complex health situations and follow care protocols.
- Document clinical findings and communicate effectively with the team.
- Engage in advanced care discussions and patient education.
Requirements
- Active certification (ANCC/AANP for NPs) or NCCPA for PAs, with state licensure.
- Unrestricted license and DEA prescriptive authority.
- Reliable transportation and valid driver’s license.
- Ability to work independently in varied environments.
Preferred Skills & Experience
- 1+ years clinical experience, preferably in geriatrics, home health, or long-term care.
- Experience with complex behavioral, social, or functional patient needs.
- Proficiency with electronic medical records and healthcare technology.
- Knowledge of geriatrics, chronic illness, and end-of-life care.
