Travel Nurse Practitioner/Physician Assistant

Monogram Health is seeking a skilled Nurse Practitioner for the role of Travel Nurse Practitioner.
About the role
The Monogram Travel Nurse Practitioner provides comprehensive care for underserved patients within the Monogram Health network, focusing on improving patient health outcomes and managing chronic conditions like CKD and ESRD.
Responsibilities
- Conduct in-home assessments, including comprehensive annual wellness exams, focused screenings, and other visits
- Counsel and educate patients and families about available benefits and programs
- Maintain accurate patient records and schedule follow-up appointments
- Coordinate care with primary care providers, specialists, and ancillary services
- Prescribe medications, order tests, and collaborate with physician practices
- Facilitate preventive monitoring and schedule diagnostic testing
- Avoidable hospitalization reduction, quality of life enhancement, and patient health/satisfaction prioritization
- Participate in integrated care team meetings and close care gaps
Requirements
- Active and unrestricted RN and NP board certification
- Current and unrestricted DEA certificate or eligibility
- Ability to travel extensively by air and utilize rental vehicles
- Proficiency with medical instruments and equipment, computer-based data management programs, and medical records systems
- Effective communication skills with retail and medical partners, patients, family members, physicians, and representatives of the community
- Knowledge of disease diagnosis and prevention, ability to assess patient health status, implement a plan consistent with appropriate plan of care, and follow-up and evaluate patient status
