About the role
We are seeking a friendly and compassionate Cardiology Nurse Practitioner or Physician Assistant to join our team. As a Cardiology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with heart-related conditions.
Responsibilities
- Perform comprehensive patient assessments, including physical examinations, medical histories, and diagnostic tests
- Collaborate with cardiologists to develop and implement treatment plans for patients with cardiovascular conditions
- Administer medications, monitor patient responses, and adjust treatment plans as necessary
- Conduct cardiac stress tests, electrocardiograms (EKGs), and other diagnostic procedures
- Educate patients and their families about cardiovascular health, preventive measures, and the importance of medication adherence
- Provide counseling and emotional support to patients and their families
- Medication Management: NPs/PAs ensure residents receive the appropriate medications, monitor for side effects, and adjust dosages as needed.
- Chronic Disease Management: They provide ongoing care for residents with chronic conditions such as diabetes, heart disease, and dementia, helping to manage symptoms and improve quality of life.
- Fall Prevention: They identify residents at risk for falls and implement strategies to reduce fall incidents.
- Knowledge of CMS Regulations and NCQA HEDIS Guidelines
- Document accurate medical records to ensure compliance with OSHA and Government regulations and perform duties in accordance with site quality and safety systems
- Analyze and organize the care of critically complex patients
Requirements
- Minimum 6 months of experience in long-term care or a skilled nursing facility
- Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
- DEA License
- CPR BCLS/BLS certified
- Must hold a Master's Degree in Nursing
- Completed a degree from an accredited Nurse Practitioner program
- Reliable transportation
- Must be willing to travel to different nursing homes across North Texas
- Preferred experience in cardiology: training provided
- Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided
- Must have great organizational and time management skills
- Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
