Travel Nurse Practitioner / Physician Assistant

About the role

Deliver personalized, compassionate in-home care to patients with complex chronic conditions, primarily Chronic Kidney Disease (CKD) through End-Stage Renal Disease (ESRD), as part of an integrated care team. The position involves extensive travel within the state to perform assessments, screenings, and follow-ups, with one telehealth day per week.

Responsibilities

• Conduct in-home health evaluations and assessments
• Develop and implement patient management plans
• Counsel patients on health management and treatment options
• Collaborate with healthcare providers to coordinate care
• Prescribe medications and order diagnostic tests
• Educate patients and caregivers on disease management
• Ensure quality of care and adherence to care standards
• Participate in team meetings and care planning sessions
• Maintain accurate and timely documentation of patient records
• Facilitate closure of care gaps and improve patient outcomes

Requirements

• Active RN and Nurse Practitioner (NP) licensure in the state of practice
• Board certification as a Nurse Practitioner
• Eligibility for DEA registration
• At least 2 years of direct patient care experience
• Proficiency in using medical equipment and electronic data systems
• Excellent communication and interpersonal skills
• Ability to work autonomously and manage a flexible schedule
• Valid driver’s license and reliable access to transportation
