Travel Nurse Practitioner / Physician Assistant

Role: Deliver personalized, compassionate in-home care to patients with complex chronic conditions, primarily CKD through ESRD, as part of an integrated care team. Responsibilities include conducting health assessments, developing management plans, counseling patients, and maintaining accurate records. The position involves extensive travel within the state to perform assessments, screenings, and follow-ups, with one telehealth day per week. Key Duties: Conduct in-home evaluations, collaborate with healthcare providers, prescribe medications, order tests, educate patients, and ensure care quality. Participate in team meetings, document properly, and facilitate care gaps closure. Qualifications & Skills: Active RN and NP licensure, board certification, DEA eligibility, at least 2 years of patient care experience, proficient in medical equipment and data systems, excellent communication skills, and ability to work autonomously.
