Responsibilities

• Review medical history
• Perform physical exams, screenings, and point-of-care tests
• Identify diagnoses
• Communicate with primary care providers
• Recognize urgent situations
• Educate patients
• Address social determinants and make referrals

Qualifications

• Active licensing (NP or PA)
• Unrestricted driver's license
• Reliable transportation
• Able to navigate home environments

Preferred Skills

• Clinical experience
• Proficiency with electronic records
• Strong communication skills with geriatric or Medicare populations

Compensation

Base salary plus incentives, ranging from $109,500 to $164,000, with comprehensive benefits.
