About the Role
Perform comprehensive in-home health assessments for Medicare Advantage members to improve health outcomes and ensure seamless care coordination.
Responsibilities
- Conduct medical history reviews, physical exams, vital signs, screenings, and point-of-care tests
- Identify diagnoses, document findings, and communicate with primary care providers
- Recognize urgent situations, provide appropriate interventions, and educate members on health and medications
- Address social determinants of health and facilitate referrals
- Navigate varied home environments and perform assessments safely
Requirements
- Active licensure and certification (ANCC, AANP, NCCPA) in relevant fields
- Unrestricted driver's license and reliable transportation for home visits
- Ability to work independently in diverse settings
Preferred Qualifications
- 1+ years clinical experience
- Proficiency with EMRs
- Strong communication skills with geriatric/Medicare populations
