Schedule

Flexible scheduling from 8am–8pm, with no on-call, weekends, or holidays.

Responsibilities

• Conduct comprehensive in-home health assessments, including medical history, vital signs, screenings, and physical exams.
• Identify diagnoses and communicate findings with primary care providers.
• Recognize and respond to urgent medical situations.
• Educate members on health management and preventive care.
• Address social determinants of health.

Requirements

• Active licensure as a Nurse Practitioner (NP) or Physician Assistant (PA).
• Certification in your field.
• Reliable transportation and ability to navigate home environments.

Qualifications (Preferred)

• Clinical practice experience.
• Proficiency with electronic health records.
• Strong communication skills, particularly with geriatric populations.

Pay

Compensation ranges from $109,500–$164,000 plus benefits.
