About the Role

We are seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health assessments for Medicare Advantage members. The role focuses on preventive care, education, and closing care gaps through comprehensive evaluations, diagnosis documentation, and collaboration with primary care providers. You will also address social determinants of health by providing referrals and interventions.

Responsibilities

  • Conduct medical history reviews, physical exams, vital signs, and health screenings.
  • Identify and document diagnoses for care management.
  • Collaborate with primary care providers (PCPs) to address and close care gaps.
  • Recognize and intervene in urgent or emergent situations.
  • Educate members on health conditions, medications, and treatment compliance.
  • Address social factors impacting health and provide appropriate referrals.

Requirements

  • Active, unrestricted licensure and certification (ANCC/AANP for Nurse Practitioners, NCCPA for Physician Assistants).
  • Valid driver’s license and reliable transportation.
  • Ability to navigate home environments and perform physical assessments.

Preferred Qualifications

  • 1+ years of clinical experience.
  • Proficiency with electronic medical records (EMRs).
  • Strong communication skills, particularly with geriatric or Medicare populations.
