About the role

As a Navigator in the Participant Wellness & Safety Program, you will work directly with individuals experiencing homelessness, substance use disorders, and complex health challenges.
Responsibilities
- Build trusting, consistent relationships with participants using trauma-informed, person-centered approaches
- Connect individuals to healthcare, MAT/MOUD, harm reduction resources (including Naloxone), and treatment services
- Support participants in developing and working toward self-directed recovery and wellness goals
- Cookout referrals and “warm handoffs” with internal teams and community partners
- Conduct outreach and engagement with individuals experiencing homelessness, including those not connected to services
- Host or support HIV and Hepatitis C testing events and connect participants to follow-up care
- Maintain accurate and timely documentation in electronic health record systems and required reporting tools
- Develop crisis plans and provide de-escalation and crisis intervention as needed
- Collaborate across teams and partners to improve access to services and reduce barriers
- Contribute to a culture of dignity, safety, and equity in all participant interactions
Qualifications
- A high school diploma or GED
- At least 2 years of experience working with individuals experiencing homelessness or similar populations
- Experience supporting individuals with substance use disorders, mental health needs, or complex barriers
- Experience using electronic health records and tracking participant data
- Strong relationship-building skills with the ability to engage participants in a non-judgmental, person-centered way
- Understanding of trauma-informed care, harm reduction, and recovery-oriented approaches (or willingness to learn)
- Knowledge of community resources, housing systems, and behavioral health supports
- Ability to manage documentation, communication, and multiple priorities in a fast-paced environment
- Strong communication and collaboration skills
- Lived experience is valued and may be considered as part of experience requirements
- Valid driver's license, insurance, clean driving record, and reliable transportation
