Job Overview

This is a union position. HealthRIGHT 360 is seeking a full-time Patient Safety Navigator in San Francisco, CA.

Client Services

  • Welcome and assist patients with navigating clinical resources throughout the building
  • Coordinate care and patient flow to ensure timely provision of services
  • Provide overview of services to be rendered, including rapid testing for HIV and Hep C; tuberculosis screening; physical exams; addiction medicine services; resource center services; and establishing primary medical and dental care
  • Escorts patients from any floor to clinic appointments
  • Aids patients with required paperwork for proper enrollment in services
  • Aids patients with transportation for residential programs

Safety Responsibilities

  • Ensures and maintains a safe and orderly environment for clients, staff, and visitors
  • Directs and defuses traffic; de-escalates tense, high-pressure, and emotionally charged situations
  • Fosters a safe, respectful, and welcoming environment
  • Maintains positive, respectful, non-confrontational behaviors
  • Clears areas of crowds, debris, and out-of-place items
  • Works collaboratively with service providers and administrative staff to maintain an orderly environment

Administrative and Documentation Responsibilities

  • Aids with data collection to monitor patient flow
  • Collaborates with the Admissions Department to help clients enter behavioral health programs
  • Performs property searches and personal inventory lists
  • Aids with incident reporting, surveillance video retrieval, and grievances

Education and Knowledge, Skills and Abilities

  • High School Diploma or GED required
  • Community Health Worker or Mental Health related certificate preferred
  • Familiarity with various supportive counseling strategies and wellness and recovery principles in working with clients with mental health, substance use, or co-occurring conditions, preferred
  • Knowledge of San Francisco community behavioral and public health systems
  • At least 2-4 years of 'lived experience' with the community behavioral health system and/or criminal justice system
  • Demonstrated ability to interface well with diverse, disadvantaged populations with multiple barriers to educational and vocational success, including low literacy, learning disabilities, physical disabilities, history of incarceration, and impaired cognitive functioning
  • Proven ability to build strong relationships with colleagues, community organizations, and the broader community
  • Understanding of social determinants of health and importance of whole person care
  • Team orientation and ability to independently assume responsibility
  • Excellent verbal and written communication, planning, and time management skills
  • High degree of creative energy and the ability to adapt easily to change
  • Proficient in MS Word, Excel, PowerPoint, and Internet applications
  • Willingness to work with clients in the community
  • Ability to respond quickly and use good judgment in emergency situations
  • Good attention to detail

Desired Knowledge and Experience

  • Bilingual
  • Experience counseling for teaching adults and/or facilitating workshops
  • Experience working with drug and alcohol addictions
  • Experience working with mental health symptoms and disorders
  • Experience working with criminal justice population
