Navigator, Clinical Care

Job Profile Summary

This role focuses on evaluating, developing, recommending, implementing, and carrying out policies and procedures related to high quality patient care. It also includes performing Care Navigation duties such as guiding patients through the healthcare system and overcoming obstacles.

About the Role

An organizational support or service role or a role focusing on support of daily business activities operating in a "hands-on" environment. The majority of time is spent delivering support services under supervision.

Responsibilities

• Supports and facilitates transitions across the care continuum through telephonic patient outreach, coordination of care, referrals to providers and agencies.
• Liaises between providers and clinical resources to ensure patient involvement in decision-making for care, including post-discharge follow-up and preventing avoidable readmissions.
• Creates and distributes patient activity reports and other patient-based reports, including databases for referrals, plan-based level of care authorizations, and out-of-network activity.

Requirements

• High school diploma or GED.
• Two (2) years of experience in a clinical setting (nursing unit, care management, physician’s office).

Preferred Qualifications

• College Degree.
• Medical Assistant (MA) certification.
• Three (3) years of related experience.

• Experience with Electronic Medical Records and willingness to learn new technology and procedures.
• General computer knowledge and proficiency with Microsoft Office, Word, Excel.
