Clinical Navigator

About the role

As a Clinical Patient Care Navigator, you will have the unique opportunity to utilize your clinical background to connect with patients, families, and referral partners with our exceptional continuum of healthcare services. In this role, you will focus on strategically expanding our business within your assigned territory while strengthening existing partnerships and fostering new connections. You will play a key role in identifying and generating opportunities to grow our services for Atlas Medical, Aegis Home Health, and Aegis Hospice.

Responsibilities

• Represent our organization, building meaningful relationships, and delivering impactful presentations on Home Health and Hospice care.
• Collaborate with our team to achieve monthly goals and ensure the highest standards of service and care.
• Make a difference in the lives of those we serve.

Qualifications

• Hospice Clinical background 2+ years
• Knowledge of local Senior Care Industry, Hospital Case Management, and Skilled Nursing
• Intuitive, solution-oriented, and driven to achieve goals
• Detail-oriented, flexible, and self-motivated
• Excellent oral and written communication skills
• Valid Driver's License (clean record) & reliable transportation
• Valid AZ Fingerprint Clearance Card (we can help with this)
• Current CPR Certification (we can help with this)
• Negative TB Test (we can test)
