Midlife Care Coordinator

About the role

Serves as a first point of contact for midlife patients seeking care, services or programs.

Responsibilities

• Conducts or decipher nonclinical intake assessments (AI assisted) to gather information necessary to appropriately schedule patient with provider or service.
• Explain care pathways, service offerings and next steps in a clear and sensitive manner.
• Support patients through scheduling process and ensure timely coordination and follow up appointments.
• Work to reduce No Shows and other delays in care.
• Act as a liaison between patients and clerical teams at defined place of service.

Qualifications

• High School Diploma or GED
• 1 year Women's Health or Primary Care Office experience

Pay

Hiring Range $17.50 - $22.75/Hourly
