About the role
The Metro Peer Case Manager is a professional case management position within the HOPICS Division of SPECIAL SERVICE FOR GROUPS, Inc. This role is non-exempt and full-time (40 hours per week).
Responsibilities
- Perform immediate needs assessments with consumers to obtain information to complete a comprehensive treatment plan.
- Collaborate with a multidisciplinary team to present within internal and external entities.
- Work comfortably with homeless clients in the field.
- Aid in establishing client treatment plans that align with clinical assessments from therapists, nurses, and substance use counselors.
- Assist clients in resolving or coping with issues contributing to mental, emotional, medical, and/or substance-related problems.
- Provide peer support and life skills techniques to clients.
- Facilitate groups in coordination with other E6 team members.
- Document in accordance with guidelines established for the E6 program, including entering all notes and treatment plans into HMIS.
- Provide case management services, including solutions to immediate needs, food support, hygiene support, linkages to various resources, etc.
- Work with homeless consumers, adults with co-occurring substance abuse issues, and individuals with mental/physical/developmental disabilities.
- Participate in mandatory program, division meetings and trainings as assigned.
- Coordinate program activities to meet contractual agreements, laws, regulations, and funding requirements.
- Attend weekly supervision (individual and group).
- Assume other tasks as assigned by the E6 Program Manager.
- Report legally required issues as per the law.
- Represent the Agency and Division in a professional manner at meetings and community events.
- Maintain regular attendance, files/records on client services in compliance with HIPAA CFR42 and Other Funding Requirements for Auditing Purposes.
- Ensure all client records/documentation are submitted or a plan arranged to send them in if resigning from this position.
Qualifications
- High school diploma and/or Registered Alcohol and Drug Abuse Counselor.
- Lived experience with homelessness and experience working with the homeless (e.g., Peer Counseling training program, or volunteer outreach).
- Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
- Ability to work with homeless clients in the field.
- Effective communication skills both written and oral with collaborative entities.
- Neat and professional appearance.
- Attention to detail and punctuality required.
- Working knowledge of Microsoft Word, Excel, and Internet.
- Experience working with homeless persons with co-occurring issues (mental/health/disabilities/substances).
- Ability to work with co-occurring populations that present with any stand-alone or combination of mental illness, health conditions, and substance-related concerns.
- Professional experience working within a multidisciplinary team.
- Ability to coordinate the submission of client documentation if unable to work.
- If in recovery, a minimum of three (3) years of being drug and alcohol free is required.
- Pass a Background Clearance.
- TB test required (not more than 3 months prior to or 7 days after Date of Hire, and renewed annually thereafter).
- CPR and First Aid Certification required within 30 days of employment with company.
- Valid California ID and insurance is not required as the position is entry-level.
Skills
- Knowledge of HIV/AIDS services and testing locations.
- Ability to maintain hygiene/food/clothing supplies and track/log them.
- Ability to handle food giveaways and clothes documentation and file properly.
- Ability to maintain hygiene product inventory documentation and enter activity into HMIS system regularly.
- Ability to prepare and submit LAHSA required reports.
- Ability to build rapport with individuals and staff of various community organizations serving homeless individuals, families, low-income households, and other HOPICS target populations.
- Ability to inform, educate, and refer homeless individuals and families about HOPICS programs and services available to them.

Pay
Pay Range: ($24.00 - $27.00 per hour)
