About the role
The Metro Peer Case Manager position is part of a multidisciplinary team serving individuals experiencing homelessness. The role involves conducting immediate needs assessments, assisting in the development of individualized service and treatment plans, and coordinating services across Metro Lines A, K, and C.
Responsibilities
- Conduct immediate needs assessments with consumers
- Collaborate with multidisciplinary teams
- Provide peer support and life skills techniques
- Facilitate groups in coordination with other E6 team members
- Coordinate program activities to meet contractual agreements, laws, and funding requirements
- Assist in outreach efforts to engage and provide services to people experiencing homelessness
- Document interventions and treatments in the field
- Participate in required meetings, trainings, and collaborations
Requirements
- High school diploma and/or Registered Alcohol and Drug Abuse Counselor certification
- Lived experience with homelessness and experience working with the homeless
- Ability to work with diverse cultural, ethnic, and socio-economic backgrounds
- Attention to detail and punctuality required
- Professional experience working within a multidisciplinary team
- Ability to produce verification of Employment Eligibility and pass a Background Clearance
Qualifications
- Knowledge of Microsoft Word, Excel, and Internet
- Experience working with homeless persons with co-occurring issues
- Ability to work with co-occurring populations that present with any stand-alone or combination of mental illness, health conditions, and substance-related concerns
- Professional experience working within a multidisciplinary team
- Ability to report legal issues that surface
- Ability to produce verification of Employment Eligibility and pass a Background Clearance
- TB test required (not more than 3 months prior to or 7 days after Date of Hire, and renewed annually thereafter)
- CPR and First Aid Certification required within 30 days of employment with company

Pay
The pay range for this position is ($24.00 - $27.00 per hour).
Schedule
The position is full-time (40 hours) and non-exempt.
