Peer Case Manager- CTI Program

Full-time, Monday–Friday 8:00 am–4:30 pm; on-call rotation required.

About the role

The Peer Care Manager provides Critical Time Intervention (CTI) services from a lived-experience and non-clinical perspective. Candidates must have direct personal experience with one or more of the following: mental illness, substance use, homelessness, or trauma.

Responsibilities

  • Complete all CTI, agency, and state-mandated trainings as assigned.
  • Foster a collaborative relationship with partnering hospitals through regular meetings and effective communication.
  • Provide care coordination for a caseload of up to 15 individuals, linking them to community supports to promote stability and management of behavioral and physical health needs.
  • Maintain accurate and timely documentation in the electronic health record per CTI manual and agency policies.
  • Deliver CTI core services: person-centered planning, psychosocial rehabilitative services, crisis prevention, health services, and care coordination.
  • Participate in weekly case presentations to review caseload status, including CTI phase, transitions in care, completed activities, and planned activities.
  • Report data to the Team Lead using the Implementation Self-Assessment Form.
  • Adhere to trauma-informed principles as defined by SAMHSA’s National Center for Trauma-Informed Care.
  • Attend case conferences to troubleshoot treatment challenges and coordinate care among providers and collaterals.
  • Assist the Team Lead in maintaining an up-to-date community resources list and access instructions.
  • Engage in the CTI Learning Collaborative and the CTI Learning Community—a collaboration with the Office of Mental Health (OMH) to review progress, outcomes, and best practices.
  • Function as part of a multidisciplinary team and participate in the after-hours on-call rotation.
  • Maintain client confidentiality at all times.
  • Perform other duties as assigned or developed; management reserves the right to modify responsibilities.

Requirements

  • At least 18 years of age with a high-school diploma or equivalent.
  • Minimum of six months’ direct-care experience with individuals with serious mental illness.
  • Certified Peer Specialist through NYPSCB.org or willingness to obtain certification within six months of hire.
  • Valid driver’s license and acceptable driving record.

Pay

$23.08 per hour.
